What is an Advance Medical Directive ?

It is a notarial form used in the event that you are in a situation that renders you incapable of consenting to health care.

The form allows you to  accept or refuse certain types of care  in advance.

The form is filed with the RAMQ directly and is accessible by a doctor without consulting with the family.

The Advance Medical directive is unlike the mandate of protection in that it is registered directly unto your medical file with the RAMQ.

The Advance medical directives focuses on three situations :

  • End-of-life situation with a serious and incurable illness rendering you incapable of consenting to care
  • Severe and irreversible comatose or vegetative state
  • Severe and irreversible loss of of cognitive state resulting in advanced stage of dementia

Should you ever be in such a situation do you accept or refuse these specific types of care :

  • Cardiopulmonary resuscitation
  • Ventilator-assisted breathing or other devices
  • Dialysis
  • Forced or artificial feeding
  • Forced or artificial hydration
